Creating an assessment template

This guide explains how to create assessment templates including scoring, grading, sections, and dependent questions.
1. Creating a template
- Templates are predefined collections of questions used to assess suppliers. You can set up as many templates as you like. Templates should remain static once created so you can compare performance over time.
- Templates can be created during the creation of a new project or directly from Assessments in the Solutions area. Navigate to Assessment Templates and select Create Template from the sub‑navigation menu.
- If creating a template from the My Templates area, click Create Template.
2. Scoring type
- The first screen asks you to name the new template and select a scoring mechanism. The scoring mechanisms are:
- Risk: Low scores are good.
- Compliance: High scores are good.
- Audit.
3. Grades
- You can associate grades with a template by clicking Manage grades for your template and assigning score ranges to each grade.
- Adding grades always starts from 0 up to 100%. For example, 0–29% could score grade D and 30–59% grade C.
4. Sections and questions
- Templates are made up of sections and questions. Sections group questions and provide sub‑scores for comparison.
- For example, a farm assessment template might contain sections for Animal Welfare, Environment & Sustainability, and Worker Welfare. You can then compare suppliers on overall scores and by section.
- If a section is not applicable to all companies, you can add a tag. When issuing assessments, you can choose whether to include tagged sections.
- For example, you might tag a section “Farms only” and issue assessments in two batches: one to non‑farms without the tagged section, and one to farms with the tagged section.
- For each question you add to a section, you can choose from the following response types:
- Single response from a list: Provide options, each with a category and score.
- Multiple responses from a list: Provide options from which multiple items can be selected. Scores can be leading (highest answer used) or cumulative (all selected scores added).
- Text response: Free text, not scored.
- Numerical response: Number input, not scored.
- Percentage response: Percentage input, not scored.
- Sub‑questions: A series of sub‑questions with numeric or percentage responses, not scored.
- For example, a single response question might categorise “weekly” or “monthly” as excellent and contribute 4 points to the overall score.
- From the template screen under My Templates, you can reorder questions in a section or reorder sections by clicking the Order button.
- To edit a section or question, click the Manage drop‑down and select the desired action.
5. Dependent questions
- Questions can be included in an assessment and triggered based on a response given to a single‑response or multiple‑response question.
- First add your trigger question and set the list of responses.
- Then add your dependent question. Enter the question text and select how it should be answered. Additional options will be displayed, allowing you to select the trigger question and the response it depends on.
6. Publishing the template
- Once you are happy with the template, click Publish. The template will then be available to send for completion.
